Here is a somewhat detailled article about what I meant.
Here is a report about an attempt to find a significant D source in AGN, which was not successful.
And if it's in Wikipedia, it must be true, right?

So yeah, saying "never anywhere" was a bit over the top; I should have excluded D as an intermediate product in nucleosynthesis by stressing net production - but I think in principle, for the purposes of a quiz, the question & answer were valid.

Edit: Clarification
I see your point, Arneb. My thought was just that the BB isn't the only place that D is produced, even most of it produced since BBN was also made into heavier isotopes. Still, BBN isn't the *only* place deuterium is produced.
