One must remember that this black hole is just the record for so-called 'stellar mass black holes'. There are also thought to be super-massive black holes at the centers of galaxies that have millions to billions of solar masses. Stellar-mass black holes are primarily formed by the supernovae phase of massive stars, while super-massive black holes are formed by the extreme density at the galactic centers and then grow by subsequent 'eating' of all the nearby matter.

Maybe. They may also be primordial.

I wonder if the inflationary period and the formation of primordial (supermassive) black holes could be linked?

We are aware of these objects because they are emitting at very high wavelengths. We do not know the net energy budget.
